When Bloating Isn’t About Food: The Hidden Gut Issue Most People Miss

“Sandy” came to see me after years of chronic bloating. She had tried every diet under the sun, assuming her bloating was caused by a food intolerance. Sometimes it would clear up for a while, but it never truly resolved.

As we got into her intake session, I learned she had experienced two major bouts of food poisoning that had precipitated her bloating. She had been diagnosed with post-infectious IBS, a form of IBS that begins after an episode of acute gastroenteritis. It usually doesn’t resolve unless the underlying issue is addressed: typically SIBO/IMO that occurs after a bout of food poisoning. Read more about SIBO here and IMO here.

Sandy was feeling really discouraged, and her GI doctor had given her antibiotics to address the post-infectious IBS. They worked for a little while, but her symptoms– the debilitating bloating and now more recently, constipation– would always return, sometimes worse.

She had also tried very strict elimination diets with little relief. By the time she came to see me she was down to four foods, something I sadly see regularly. She had dumped gluten, dairy, most carbs, sugar, red meat, and pork. She ate eggs, chicken, zucchini, and rice. That was it.

I first reassured her that her bloating was treatable, but not with food alone. There was something else at play here that was causing her bloating, and by addressing the underlying cause, she would be able to tolerate more foods over time.

I recommended breath testing for SIBO and a GI MAP stool test to assess any dysbiosis in the large intestine. Sure enough, her breath test showed high levels of methane gas which was causing her bloating and constipation. Now that we knew what we were dealing with, I could recommend an anti-microbial protocol to address the methanogen overgrowth in the small intestine.

If you’ve tried cutting out gluten, dairy, sugar, carbs, or all of the above, and you’re still bloated, this may come as a relief to hear: Your bloating may not be about food at all.

If you have chronic bloating not caused by food, like Sandy, the issue is often happening inside you small or large intestine (or both!) rather than on your plate.

Why elimination diets don’t fix chronic bloating

Elimination diets are often the first thing people try when bloating becomes a daily issue. And sometimes they help, temporarily.

But if your bloating:

  • comes back no matter what you eat

  • shows up even when you’ve eliminated food groups like gluten & dairy

  • gets worse as the day goes on

  • hasn’t improved despite years of restriction

then food sensitivity is unlikely to be the root cause. In those cases, the problem is usually how food is being digested, not the food itself.

Chronic bloating not caused by food: what’s really happening

When digestion is working well, food moves smoothly from the stomach into the small intestine, where nutrients are absorbed, and then into the large intestine where the fibers are fermented and the waste is released.

But this process can be affected by bacteria colonizing in the small intestine where they don’t belong. The small intestine should be a relatively sterile environment, free from the bacteria that live in the large intestine and digest your food for you. So when this bacteria migrate from the large to small intestine, symptoms such as pain, bloating, belching, abdominal distention, reflux, constipation and/or diarrhea result as the bacteria ferment the carbohydrate fibers you eat.

This migration happens when your migrating motor complex, the peristaltic cleansing waves that sweep bacteria back down to the large intestine where they belong, gets paralyzed. The biggest cause? Food poisoning.

When the bacteria start fermenting carbohydrate fibers (their preferred food source) in the small intestine, the fermentation creates gas. The gas creates pressure. The pressure creates bloating. And the methane and/or hydrogen gases released as a result of the fermentation cause constipation and/or diarrhea.

This is the pattern behind gut dysbiosis and very commonly SIBO (Small Intestinal Bacterial Overgrowth) or IMO (intestinal methanogen overgrowth).

Why this causes bloating no matter what you eat

When fermentation is happening in the wrong place, almost any food can trigger symptoms.

That’s why people with this pattern often say:

  • “Everything makes me bloated.”

  • “I feel worse after meals even when I eat slowly.”

  • “I’ve run out of foods I can tolerate.”

At that point, further restriction doesn’t solve the problem. It often makes it worse by weakening digestion and slowing gut motility even more. Restrictive diets also starve out your colonies of beneficial bacteria, worsening bloating because you don’t have the bacterial diversity to properly digest your food for you.

Many people with this type of bloating are told they have one or more of the following:

  • food sensitivities

  • IBS

  • stress

  • candida (can also be present but not the main cause)

But these conditions don’t explain why the bloating is happening. IBS in particular is a “diagnosis of exclusion” when a cause of chronic GI issues or chronic constipation or diarrhea can’t be pinpointed. And ~80% of IBS cases are actually caused by SIBO/IMO!

SIBO and dysbiosis are frequently missed because:

  • symptoms overlap with IBS

  • testing isn’t always done

  • treatment often focuses on food instead of gut function

The result? People stay stuck in cycles of bloating, restriction, and relapse.

The good news: This type of bloating is very fixable

When bloating isn’t about food, relief comes from addressing things like:

  • gut motility

  • small intestine health

  • the gut environment

  • nervous system regulation

Once those pieces are supported, many people find they can finally:

  • eat a wider variety of foods

  • stop obsessing over every food

  • feel comfortable in their body again

  • find relief from chronic bloating!
